- Medical records and treatment information created in the course of care, which are governed by applicable state and federal medical privacy law and by the practice's separate patient-facing policies.
Please do not submit detailed clinical or medical information through the website contact form. It is a standard web form and is not a secure or confidential channel. Share clinical information through the secure client portal or during a scheduled appointment.

Information you provide. When you submit the contact or appointment request form, you may provide your name, email address, telephone number, and any information you choose to include in the message field. You may also provide information if you email or call the practice directly.
Information collected automatically. Like most websites, this site and its hosting provider may automatically collect technical information such as your IP address, browser type, device type, referring page, pages viewed, and the dates and times of your visits. This information is collected through cookies and similar technologies and is used in aggregate to understand site usage and maintain site security.
